Employee Status Change Form questions?
Here's the form if you need to see a copy:   Employee Status Change Form

Check with your steward before submitting it!

Note on partial FTE reductions -
If you are partially reduced at the site, Selection #5 is the most flexible choice - you can seek full time and/or partial FTE positions as they come available later.    You are guaranteed your full contract FTE in the District, even though a particular site doesn't have enough FTE available. There's no guarantee your partial will be matched later, but the possibility remains available under #5.  Selection #4 will completely excess from the site now - this would be correct if you know that you would not be interested in finding a matching partial.

Contact MFT if you have any special circumstances!

Keep a copy in case you have a question about it.

SL Donation - Please consider helping out a colleague!
Thanks for your overwhelming support for Tani!    Her needs have been met and she is extremely thankful for the donations that were submitted!

If you are interested in supporting others, we currently still have two recipients in need of donations that have been determined eligible for donations after a review of their cases:
SL Donation form - KellyS  - 21 days needed

Privacy is maintained unless the recipients are comfortable sharing more.

Your participation is completely voluntary, but will make a difference!

If your steward circulated the information to you, check with your steward to see if they're collecting the forms.

Please read carefully, fill out carefully and sign before you FAX or MAIL it once to MFT (Fax # is 612-529-0539).  MFT will follow up with the District after that.  To avoid duplication, please do not fax or mail it to the District directly.

Thanks for considering the needs of your colleagues!

Think you may need to go on medical leave?
Medical Leave of Absence.  You need to be on an approved medical leave be away for over 10 consecutive duty days.

Questions about Medical Leave or Medical Leave Approval?
Contact Heather Berggren at Heather.Berggren@mpls.k12.mn.us or 668-0510 to clarify what paperwork is required and to get your leave formally approved.

Sick Leave Days (Balance).  Your sick leave can be applied for up to 90 duty days.  To find your balance, check your paystub for the number of SL hours and divide that number by 7.5 to get SL days.  Subtract one day for each month remaining in the school year because those days have not been earned yet (they are credited to your SL balance at the beginning of the school year).

How do Sick Leave, Sick Leave Pool, and/or Long-Term Disability work?
Sick Leave Pool, Sick Leave Donations.  If you don't have enough Sick Leave days to cover your absence during the first 90 duty days on Medical Leave, then you should submit your medical information to see if you are eligible for Sick Leave Pool (if already a member of the Pool) or Sick Leave Donations.  This benefit pays 80% of your salary and can pay you if you will exhaust your own sick leave. There's a 5 unpaid duty day waiting period before it can kick in.  If you are not a member of the Pool, you still need to have your medical information reviewed and determined eligible before your colleagues can donate if you are not a member of the pool.  If approved, the District and Union will help locate donors.

Having a workplace conflict that threatens to derail your career?
The Roadmap to Resolution program is the name of the union-sponsored program provided through Education Minnesota that provides facilitation for conflict resolution.  This voluntary process is confidential, takes place outside of the District and is voluntary on the part of both members.  You are encouraged to attempt self-resolution, but it not necessary that you have done so before making a request for facilitation.

A facilitator will contact both members to seek dates/times outside of the school day - you should plan on a commitment of two hours.  Keep in mind that the facilitator who contacts you will be looking to setup the date/time, clarify the process if you have questions, but is not seeking to collect background information about the conflict at this time.  There will be two co-facilitators who will be present when you convene and they will work together to lead you and the other member through the process together.

The link to the Education Minnesota webpage with more background information, request forms and other resources is at:

Click:  Roadmap to Resolution (Ed Minn weblink)

The facilitators will destroy any notes they've kept (except for the outcome which is tracked for program reasons) once your meeting has ended, nor will any information from this process be provided to MFT or the District, it is completely confidential.
